Please select the branch office you will be transacting business through and review the appropriate Brokers Agreement. Once you have reviewed the agreement please return it signed and dated to the marketing department or contact person at Insurance Innovators. In addition to this agreement, we will need a copy of your Errors & Omissions Declaration Page (premiums can be removed for privacy, but must show limits) or a certificate of insurance showing the limits, as well a copy of all appropriate state resident and non-resident retail license in which you plan on transacting business.
